Referral Forms

A referral form is just a way to reach out and get in contact with each other. We can better understand your needs and you can better understand what we provide. 

We will be notified and process the form. Then one of our friendly team members will be in contact with you shortly with relevant information. We may require further details to complete your form. 

A referral form can be submitted by: 

• NDIS Participants
• Participant family members or guardian plan nominees
• Support Coordinators
• Other actors with the approval of the participant

It is imperative to offer choice and control to the participant. Therefore, consent from them or their relevant guardian must be obtained before a referral form is completed.

No people who do not have NDIS support may still receive help from HOP.

It is possible to engage our services through an insurance company. If you're not sure confirm with your insurance company and ask if you're eligible for HOP support. 

No this is more of an inquiry to determine how our services can help you.  You are not obliged to engage our services and we want you to find the most appropriate care for your needs.
